School-Based Case Management

SNBC’s School-Based Case Manager collaborates with A.P. Giannini Middle School staff and Sunset Youth Services to work with students with special needs and provide them with support and prevention services. Clients referred to the program are high risk or at risk for truancy, academic struggles, behavioral issues, and are prone to getting caught up in the juvenile justice system. SNBC’s School-Based Case Manager serves as an adult role model and is a consistent, stable ally to the youth clients. Services include:

Probational Support
Provides support to the client and family by attending appointments, court dates, and assisting the client and family in understanding the probational process.

Weekly Boys’ Group
Held on Tuesdays at lunchtime, this is a place for young males to discuss real life issues surrounding their neighborhoods, including current events and the effect they have on youth.

Gang Prevention
This is directed to APG youth flirting with the idea of the gangster lifestyle. The School-Based Case Manager talks to youth about the dangers of living a street life in hopes of preventing youth from joining gangs, committing crimes, suffering bodily injuries or death.

Life Skills
To help young men survive and strive, the School-Based Case Manager informs young males on proper etiquette, communication, healthy choices, relationships, Juvenile Justice laws, financial budgeting, and job readiness.

Conflict Mediation
Disputes between A.P. Giannini youth can be brought to the School-Based Case Manager and heard in a safe, productive space.

Service & Program Referrals
If clients require additional services, the SNBC School-Based Case Manager assists youth and their families in connecting with other program and service providers.

Lunchtime Academic Support
Clients can come in on Thursdays at lunchtime to receive assistance with homework and class assignments. The School-Based Case Manager will also check in with teachers about the client’s academic progress.
